Dr. Marc Toglia is one of the most experienced surgeons in the Philadelphia Region performing the minimally
invasive pubovaginal sling known as TVT for the treatment of stress urinary incontinence. He has performed
over 1,000 procedures with the TVT family of products, including the original TVT procedure, TVT obturator,
and the new TVT Secur procedure. These procedures have revolutionized the surgical correction of SUI, are
performed as an outpatient procedure and associated with high levels of patient satisfaction. Minimally
invasive slings can be performed in the vast majority of patients, including women who have failed previous
surgical repair attempts. Dr. Toglia performs these procedure under IV sedation and patients are discharged
the same day. The vast majority of patients are discharged home without a catheter and note immediate
resolution of their stress urinary incontinence.

| Experiencing mesh related complications or problems? |

Synthetic mesh and biologic grafts are commonly used in the surgical correction of both female stress
urinary incontinence and reconstructive vaginal surgery. Their use is infrequently associated with long
lasting complications such as chronic pelvic pain, difficulty voiding, pain with sexual intercourse, and
other mesh related complications including mesh exposure. Dr.'s Toglia and Fagan are experienced in the
management of these problems, including mesh revision surgery, as well as more conservative therapy including
Trigger Point injections and physical therapy such as myofascial release.

| Minimally invasive options for heavy menstrual bleeding |
|
|
Heavy menstrual bleeding, or menorrhagia, is a common problem experienced by women. Minimally invasive
therapy is commonly used in women once they have completed child bearing. The "gold standard" for treatment
is an outpatient procedure known as Hysteroscopic endometrial resection or ablation. This procedure, performed
in an outpatient setting, removes the basal layer of the endometrium through surgical resection. Other causes
of heavy bleeding, including submucus fibroids, can also be treated by this modality. Dr.'s Toglia and Fagan
have significant experience utilizing the bipolar resectoscope, a state of the art instrument which offers
significant safety advantages over the older gynecologic resectoscope. Endometrial resection can be done as
the primary procedure to treat menorrhagia, or to treat women who have failed global endometrial ablation, or
have a uterine cavity that is too large for this technique.

| New Surgery for Stress Urinary Incontinence |

TVT Secur is the third generation, minimally invasive mid-urethral sling introduced by Gynecare, creators of
the original TVT sling. The system has been uniquely designed for less pain and easier recovery, and there
are no external skin incisions. "Our initial experience has been extremely positive" reports Dr. Toglia.
"The procedure is performed with a single one-quarter inch incision in about 20 minutes. Most patients have been
ready to leave the hospital within two hours, and most have taken nothing more than Motrin for discomfort.
It is certainly the least invasive of the Current procedures, which I suspect will reduce some of the reported
complications and minimize post-operative pain."
